The physician will work closely with advance practice providers, social workers, case managers and other members of our health care team. They will also participate in multidisciplinary conferences that focus on the frailest members. He/she will have access to advanced technology, including a robust EMR, and a proprietary decision support tool that synthesizes data from a variety of sources into actionable alerts that drive care.

Job Duties/Responsibilities:

Provide evidence based primary care to patients with emphasis on seniors Complete annual wellness exams for both new and existing patients Minor office procedures as indicated Coordinate care with specialists and other ancillary providers Ensure all patients have advanced care planning. Educates patients on various health related topics, such as disease management, medications, healthy lifestyle, end of life care, etc. Participate and at times lead interdisciplinary team case conferences Focus on population health for assigned patient panel Work towards HEDIS and CAHPS goals as established Provide outstanding patient experience and care access Maintain a customer-driven passion for excellence and commitment to innovation and implementation of ideas that improve the healthcare of the patient population Contribute to QI, peer review and special projects as needed Comply with all HIPAA regulations and maintain security of protected health information (PHI) Supervisory Responsibilities:

Assist in supervision of medical assistants and other office staff. May directly supervise advance practice clinician(s).

Job Requirements:

Experience:

Required: 10 years of experience as an Internist or Family Practice clinician

Preferred: 1. 1-3 Years of managed care experience preferred. 2.

Basic understanding of the UM/QM process.

Education:

Required: MD./ D.O.

Training:

Required:

Completion of residency program in the United States Internal Medicine or Family Medicine trained

Licensure:

Required:

Board Certification Medical license in the state which you will be practicing

Clinical confidence and friendly bedside manner Flexibility and willingness to learn new systems and processes Strong communication skills

Preferred:

Substantial knowledge across medical subspecialties Bilingual (English and Spanish)
